Melanoma is the most aggressive and deadly form of skin cancer due to its high propensity to metastasize to distant organs. Significant progress has been made in the last few decades in melanoma therapeutics, most notably in targeted therapy and immunotherapy. These approaches have greatly improved treatment response outcomes; however, they remain limited in their abilities to hinder disease progression due, in part, to the onset of acquired resistance. In parallel, intrinsic resistance to therapy remains an issue to be resolved. In this review, we summarize currently available therapeutic options for melanoma treatment and focus on possible mechanisms that drive therapeutic resistance. A better understanding of therapy resistance will provide improved rational strategies to overcome these obstacles.
黑色素瘤因其极易向远端器官转移的特性，成为最具侵袭性和致命性的皮肤癌类型。过去几十年间，黑色素瘤治疗领域取得了显著进展，其中靶向治疗和免疫治疗的突破尤为突出。这些治疗方法显著提升了治疗应答效果，但由于获得性耐药的出现等因素，其在遏制疾病进展方面仍存在局限。与此同时，固有耐药性仍是亟待解决的临床难题。本文综述了当前黑色素瘤的主要治疗策略，并着重探讨驱动治疗耐药性的潜在机制。深入理解治疗耐药性将有助于制定更合理的策略来突破现有治疗瓶颈。
