The first half of 2022 saw the publication of several major research advances in image-based models and artificial intelligence applications to optimize treatment strategies for high-grade gliomas, the deadliest brain tumors. We review them and discuss the barriers that delay their entry into clinical practice; particularly, the small sample size and the heterogeneity of the study designs and methodologies used. We will also write about the poor and late palliation that patients suffering from high-grade glioma can count on at the end of life, as well as the current legislative instruments, with particular reference to Italy. We suggest measures to accelerate the gradual progress in image-based models and end of life care for patients with high-grade glioma.
2022年上半年,多项基于影像模型与人工智能技术优化高级别胶质瘤(致死率最高的脑肿瘤)治疗策略的重大研究进展相继发表。本文系统评述了这些研究成果,并探讨了阻碍其进入临床实践的主要障碍,尤其是样本量不足以及研究设计与方法学存在的异质性。同时,我们将阐述高级别胶质瘤患者在生命终末期所能获得的姑息治疗资源匮乏且滞后的问题,并对现行法律工具(特别是意大利的相关法规)进行分析。最后,本文提出加速推进基于影像模型的技术发展及改善高级别胶质瘤患者临终关怀服务的具体建议。
