Osteosarcoma (OS) is a heterogeneous, highly metastatic bone malignancy in children and adolescents. Despite advancements in multimodal treatment strategies, the prognosis for patients with metastatic or recurrent disease has not improved significantly in the last four decades. OS is a highly heterogeneous tumor; its genetic background and the mechanism of oncogenesis are not well defined. Unfortunately, no effective molecular targeted therapy is currently available for this disease. Understanding osteosarcoma’s tumor microenvironment (TME) has recently gained much interest among scientists hoping to provide valuable insights into tumor heterogeneity, progression, metastasis, and the identification of novel therapeutic avenues. Here, we review the current understanding of the TME of OS, including different cellular and noncellular components, their crosstalk with OS tumor cells, and their involvement in tumor progression and metastasis. We also highlight past/current clinical trials targeting the TME of OS for effective therapies and potential future therapeutic strategies with negligible adverse effects.
骨肉瘤(OS)是一种发生于儿童和青少年的异质性、高转移性恶性骨肿瘤。尽管多模式治疗策略有所进展,但近四十年来转移性或复发性骨肉瘤患者的预后并未得到显著改善。骨肉瘤具有高度异质性,其遗传背景与致癌机制尚未明确。目前,该疾病仍缺乏有效的分子靶向治疗方案。近年来,对骨肉瘤肿瘤微环境(TME)的研究日益受到科学界关注,这有望为深入理解肿瘤异质性、进展、转移机制及发现新型治疗途径提供重要线索。本文系统综述了当前对骨肉瘤肿瘤微环境的认知,涵盖其细胞与非细胞组分、这些组分与骨肉瘤细胞的相互作用及其在肿瘤进展与转移中的作用。同时,我们重点梳理了既往及当前针对骨肉瘤肿瘤微环境的临床试验,并展望了未来可能实现高效低毒治疗的新策略。
